Duane, I had asked him what it would cost to make another one and he said the man-hours that it took to machine the guide and the cost of the stock would put the price at around $300 to $400 due to the fact that all of the machine work would has to be done manually. He did it for me as a favor and I owe him big time for it.
I have the Parasolid CAD File (also known as a Modeller Transmit File) so that a drill guide jig can be made on a CNC machine if you would like me to send it to you. I was quoted between $200 to $250, which included materials, to have a machine shop make one on their CNC.

GoKraut is the one who emailed me the CNC file. The $200 was what I was quoted by a local machine shop here in Renton, WA to have them machine me the part and included all materials. The price of $300 to $400 was what my friend said it would cost for them to buy the bar stock, drill bits, and the man-hours involved in manually machining the part since he didn't have a CNC milling machine.
I'm sure you can find a machine shop locally that can handle the template file and make the part on the CNC mill. I think my friend made the part for me as a favor and also because he considered it a challenge to try and make it, he's just that kind of person.
aside from it being hardened i don't see what is so expensive? its a round piece of metal with 7 holes in it?
