Is this common?
A third of the belt was eaten off, I thought it may have been rubbing against the guard, but then today I took off the crank pulley. Does this happen a lot?
Could it be the IP bracket is bent?

I would say you have something out of alignment. Bent IP brackets, bad intermediete bearings, bent tensioner bolt.....I think quickTD told us that belts run away from the initial point of contact when there are alignment problems.

I put a straightedge across all the pulleys and they all line up straight.
I think the problem is on the intermediate shaft pulley. A little nick in the upright/guide part catches the belt. The belt starts riding on top of that causing the crank pulley to become the guide.
