Title: Inserts Falling Out

I had a 5 cylinder Audi N/A diesel that had a head fall apart about 2 years ago.  At the time I only noticed that one of the inserts had fallen out and mashed one of the pistons.  I scrapped that engine, but now I have a little time to clean up some of the pieces to sell them.  I found that only two of the pistons were in usable shape.  One was totaly mashed, and one had a big dent from hitting a loose insert, and the other had a small crater blown out of it in the same area that the insert would hit.  What caused this?  Would setting the timing way to far in advance (I didn't have a dial gauge then) cause the inserts to be blown out of the head?

I think overheating is one cause. I bet having overly advanced timing wouldn't help matters either.
