My clutch disc needs to be replaced but I only have 25,000 kms on the setup I was wondering if it'd be ok if I just replaced the disc. And can you get the pressure plates machined? I have an act 6 puck sprung hub clutch. I think the clutch should've lasted longer even though its been through its fair share of abuse but is that how long racing clutches last?

most of the time, i replace JUST THE DISK, and dress the flywheel and pressure plate with rough emery cloth.. just to get the glazing off the metal..
ive done this many MANY times.. and never had an issue with it.. as long as you get all the glazing, and hot spots off the PP and FW, then everything should be just fine..
on rigs that i didnt care about, ive just taken and replaced the disk, and then put it back together.. no dressing of any parts.. it worked fine, but i could tell that i didnt dress the surfaces..

i too have jut replaced the disk on two clutch jobs. Quick and dirty as i like to call those types of replacements.
However if you know for a fact everything is new, why replace it right? Maybe it is slipping because you have a trans fluid leak? Did you do its seals when you had this clutch and stuff out 25k ago?
It really depends on what shape it's in. If not scored or spotted and the springs are still good[ just stand on it and try to feel resistance when pushed] , you can probably reuse it, esp. with the low milage
Many many times we have reused the PP. I once killed a disc in 3k darn oil pressure sending unit leaking and me being too lazy to put the bellhousing plug in when i was done the first time!
i did it before and it worked fine, my clutch did slip after i had monkeyed with boost and fueling, but only after hard launches. like ror said, deglaze both surfaces
